Member of the Civilian Component



 
Application for a MANDATORY SHAPE (AMIS) IDENTITY CARD FOR A MEMBER OF THE CIVILIAN COMPONENT (NATO SOFA) OR CONSULTANT hired by SHAPE or any other organization supported or administered by SHAPE.
 
Upon presenting yourself for registration at the BSG Family Support and Central Processing Centre (FSCEP) in building 210, Rm 109-111, please bring your original passport or another national ID (valid for at least three months) for reference purposes. Besides that, the following documents should be submitted:
 
  • an original electronically completed SF 94 signed by either the respective National Military Representative (NMR) himself or any other individual nominated by the NMR and certified by an official NMR stamp, or for civilians signed and stamped by your HR Civilian Personnel Branch  
  • an original completed SF1 1 Aug 08 (SHAPE Security Pass Application) signed/stamped by the responsible Division Security Officer (DSO)
  • an original completed ACE Form 107 R Jul 2003 (Personnel Security Clearance) signed/stamped by the responsible DSO
  • an original NATO Security Clearance Certificate made out by the respective NMR office or the HR Civilian Personnel Branch 
These documents serve not only for the determination of your legal status and level of entitlements, but also to determine the correct security level on the respective SHAPE (AMIS) ID Card and to secure your respective security access. Once your registration has been completed based upon the above documents, you will receive your SHAPE (AMIS) ID Card with an integrated ration card - if applicable - for tax free purchases on the compound.
You can also apply for a SHAPE driver's license if you have a national drivers license which is recognized by Belgium. 
 
Application for a MANDATORY BELGIAN PROTOCOL IDENTITY CARD FOR A MEMBER OF THE CIVILIAN COMPONENT (NATO SOFA) hired by SHAPE or any other organization supported or administered by SHAPE (only possible once you have a Belgian address)
 
This Protocol ID Card is the valid proof of residency in Belgium released by the Ministry of Foreign Affairs for special foreigners like yourself.
In order to apply, you will have to submit to the FSCEP the following documents  (pls bring your copies with you since copies cannot be made at our offices!!):  
 
  • two colour copies of your passport or national ID (valid for at least 3 months),  if required with a valid Schengen visa 
  • two colour copies of your marriage certificate or an official certificate of legal partnership, accompanied with a translation (Marriage Certificate Form Annex B) stamped and signed by your NMR or HR Civilian Personnel Branch for authentic and original, if the document is not in the English, French, German or Dutch language
  • two copies of your birth certificate if you are single,  accompanied with a translation (Birth Certificate Form Annex A) stamped and signed by your NMR or HR Civilian Personnel Branch for authentic and original, if the document is not in the English, French, German or Dutch language
  • passport pictures - facility at Registration office 

Your application will be submitted to the Ministry of Foreign Affairs. It will take approximately 6 weeks before the card will be issued.

Whenever there is a change of address, please report directly to the FSCEP Registration office. No SF-94 will be required.
